Referring a patient?
If you are an orthopedist, primary care physician, OB-GYN, or other referring provider, we appreciate your referral. Our therapists communicate directly with referring providers on patient progress, treatment status, and return-to-activity clearance.
Referrals can be sent via fax, secure email, or through the contact form above with "Physician referral" selected as the topic. We will coordinate scheduling directly with the patient.
